To request a refill, adjustment, or new medication please dial (630) 270-7717 and press 2 to leave a message. Please always be sure to call before your medication runs out as request can take up to 72-hours to complete. Certain new medications may require routine follow up appointments to ensure continued efficacy and all controlled substances require a visit every three months.
Please bring a photo ID, a list of all medications you are currently taking and your insurance card or copy of your card. If your insurance requires a co-pay/deductible, please be prepared to pay that at the time of your visit. In addition, it is helpful to write down a list of questions for your provider so that we can ensure that we answer all of your questions in the therapy room.
Please call the appropriate office location as soon as you realize you will not be able to keep your appointment. We prefer all cancellations be made at least 24 hours prior to your scheduled appointment time. This will allow us to give another patient the opportunity to be seen. If you cancel the day of your appointment there is a $50 cancellation fee.
Our registration staff will ask you to fill out registration and health history forms. We ask that new patients arrive 30 mins before scheduled appointment time to complete your paperwork. These forms may be accessed on our website under patient center. New patient visits are schedule for one hour.
If you need paperwork to be completed please speak to the front desk. Due to the many requests and time it takes to accurately fill out paperwork your forms may take up to 7 business days to be completed. Additionally, some paperwork might require an appointment with your doctor. All paperwork that needs to be completed by a physician has a service fee.
Telemedicine services allow you to see select doctors and therapists via a secure video on your phone or computer for your appointment. Telemedicine visits are a great alternative that allow you to visit with your provider without ever leaving the comfort of your home! This service is offered to established patients. Most insurance plans cover the video appointment.
